Diabetes Prevention and Education
We can help you take control of your health.
The Health Department is working to help prevent type 2 diabetes and manage diabetes in those who have been diagnosed by providing education and prevention classes to the community.
Prevent or delay the onset of type 2 diabetes with our Diabetes Prevention Program.
Nearly one out of three Americans has prediabetes, most do not know it.
When you have prediabetes your blood glucose (sugar) level is higher than normal but not high enough to be diagnosed as diabetes. High blood sugar raises your risk of type 2 diabetes, heart disease, and stroke.
Without weight loss or moderate physical activity, many people with prediabetes can develop type 2 diabetes within 5 years.
Your chances of having prediabetes go up if you:
- Are 45 or older
- Are Black, Hispanic/Latino, American Indian, Asian American, or Pacific Islander
- Have a parent, brother or sister with diabetes
- Are overweight
- Are physically inactive
- Have high blood pressure or take medicine for high blood pressure
- Have low HDL cholesterol and/or high triglycerides
- Had diabetes during pregnancy
- Have been diagnosed with Polycystic Ovary Syndrome
It takes just one minute to know your risk. Take the test now!
Or get screened at our health centers.
If it turns out you are high risk for developing diabetes or prediabetic, take charge of your health and sign up for our diabetes prevention program!
Manage type 2 diabetes with our Diabetes and Nutrition Education Program.
If you’ve been diagnosed with type 2 diabetes, learning how to maintain a healthy diet and add fitness to your daily routine can help manage type 2 diabetes.
Exercise and diet play an important role in helping keep your blood sugar down and can help prevent serious complications such as heart attack, stroke, blindness, kidney failure, or loss of toes, feet or legs.
